Advanced Nurse Practitioner/ Allied Health Professional (Nursing and Care Homes)

Cirencester, Tetbury

Contract: Permanent

An exciting opportunity has arisen at our established and busy practice for an experienced Advanced Nurse Practitioner/Allied Health Professional to join our team at the Phoenix Health Group.

The Phoenix Health Group has been offering an enhanced care home service for nearly two years. The role has moved away from traditional reactive models of care delivery; towards proactive care that centres on the needs of the individual resident and their families. The overall aim is to improve the quality of care they receive, ensuring optimum and consistent access to, and provision of NHS services.

The improved collaborative working between Healthcare Professionals, Care Home Partners and the use of urgent community services in the area; has resulted in more positive outcomes for the residents in terms of their (and their family) personal preferences and avoidance of inappropriate emergency admissions to acute and community hospitals.

The post holder will be an experienced Nurseor Allied Health Professional, who, acting within their professional boundaries, will provide personalised care for each resident of their allocated care home, from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their continued care.  They will demonstrate safe, clinical decision-making and expert care for residents in care homes.  They will work collaboratively with other community health care providers, hospitals and the care home partners to meet the needs of residents, supporting the delivery of policy and procedures, and providing nurse leadership as required.
