As a Care Planner, you will be responsible for coordinating and managing healthcare services, ensuring that clients receive the highest standard of care. You will work closely with clients, healthcare professionals, and internal teams to develop and implement personalised care plans while ensuring efficient service delivery.
Key Responsibilities
- Plan, coordinate, and manage care services across various healthcare settings.
- Work closely with clients, families, and healthcare professionals to assess needs and create tailored care plans.
- Ensure timely and efficient service delivery, often at short notice.
- Maintain accurate records, ensuring compliance with industry regulations and company policies.
- Communicate effectively with multidisciplinary teams, including NHS staff, private healthcare providers, and community care professionals.
- Monitor and review care plans to ensure high standards and continuity of care.
- Support staff allocation, ensuring the right professionals are matched to client needs.
- Respond promptly and professionally to any concerns, queries, or changes in care requirements.
- Participate in the on-call rota (1 night per week and a shared weekend per month) to ensure continuity of service.
- Experience in care planning, healthcare coordination, or a similar role.
- Strong understanding of NHS and private healthcare services.
- Excellent organisational and time-management skills.
- Ability to work under pressure and respond to urgent requests.
- Strong communication and interpersonal skills.
- Knowledge of relevant healthcare regulations and compliance.
- Proficiency in IT systems for care planning and record-keeping.
- Salary: Competitive
- Holiday: 25 days annual leave plus bank holidays
- Working Hours: 37.5 hours per week
- On-call responsibilities: 1 night per week and 1 shared weekend per month
