About the role
To work as part of a multi-disciplinary internal team, external stakeholders (including families and carers and local authorities). Proactively promote and provide transition related Information Advice and Guidance that contributes to student transition planning for all year groups at Ullenwood and other locations. To research opportunities for learners including maintaining a database of resources to support life after college.
To serve as the primary point of contact for all key stakeholders, including families, carers, and local authorities for a selected group of students. You will proactively promote and provide comprehensive transition information, advice, and guidance to facilitate effective student transition planning alongside other team members.
PRIMARY TASKS:
*To effectively research key information advice and guidance in line with college and the National Frameworks relating to transition.
*To work with a caseload of individual students, providing transition information, advice and guidance in line with national frameworks.
*To liaise with families and carers of a selected group students and their families from baseline and at pivotal points to the end of their placement at NSC.
*To lead on the creation of a personalised and aspirational transition plan throughout their learner journey for a selected group of Year 1 students.
*To develop and maintain the IAG information and resource areas including the LAC power app and provide information in accessible formats to meet the needs of individual learners.
*To contribute to the reporting and recording of achievements on the current systems including the annual review process, to ensure a multidisciplinary approach to support individual students’ outcomes and record time spent on tasks.
*To support with preparation for regular IAG standard assessments such as Matrix, liaising with internal multidisciplinary contacts in order to gain evidence of IAG good practice.
*To work with key staff including PLCs/PLCTs, Job Coaches, Continuous Placement Officer, Careers Lead and others to ensure transition outcomes are in line with Education, Health and Care Plans to meet the individual needs of each learner.
*To contribute to student transition events, forums, reviews, focus and team meetings as required when capacity allows.
*To undertake research and any other duties as required to support the Transition Support Team.
*To fulfil requirements of college appraisal and support and development systems, supporting effective practice.
*To carry out such other duties as may be required by the Principal and/or the Management Team of the Learning Programmes Department.
