Head Care Coordinator

Job Purpose
The Head Care Coordinator is responsible for leading the care coordination function to ensure safe, efficient and person-centred service delivery across all branches. The role is the primary operational contact for all new business referrals, overseeing the onboarding of new service users from enquiry through to successful commencement of care. The post holder is also responsible for monitoring rota efficiency, supporting Care Coordinators with scheduling, and developing standardised templates and guidance for support workers.
Key Responsibilities
· Act as the primary operational contact for all new business referrals.
· Coordinate referrals from Local Authorities, NHS, private clients and other referral sources.
· Oversee mobilisation of all new care packages from referral to commencement.
· Liaise with Care Managers regarding assessments and care planning.
· Lead and support the Care Coordinator team.
· Monitor rota efficiency including travel time, waiting time, mileage, continuity and utilisation.
· Identify improvements to maximise capacity and reduce unnecessary travel.
· Coordinate staffing for new packages and ensure all documentation is completed before care starts.
· Act as the communication link between Care Managers, Recruitment, Field Care Supervisors, Support Workers and external professionals.
· Support quality assurance, governance and CQC compliance.
· Produce operational reports on referrals, package starts, rota efficiency, capacity and KPIs.
Person Specification
· Experience in domiciliary care coordination.
· Strong organisational and scheduling skills.
· Excellent communication and leadership abilities.
· Good IT skills and experience with care management systems, Access ideally.
· Knowledge of CQC Fundamental Standards.
· Full UK driving licence (desirable depending on service).

Salary: £32,000-£35,000
Hours Per Week: 40 hrs
