SEND and SEMH Transitions Lead

SEND and SEMH Transitions Lead

Gloucester

Main Duties:
We are seeking an experienced SEND Transitions Lead to join our passionate team at Bridge Training. This is a vital role for someone who can deliver outstanding transition experiences for SEND learners into a variety of programmes, and as they progress on to future destinations.

Bridge Training is not your typical education provider. Since 1989, we’ve created alternative educational pathways for young people across Gloucestershire who may not thrive in mainstream settings. Our focus is on rebuilding confidence, re-engaging learners, and delivering real, impactful change through:
• Vocational training
• Functional Maths and English
• Small class sizes and tailored learning
• Supportive, flexible teaching environments

As SEND Transitions Lead, you will:

• Deliver personalised transition experiences for EHCP and SEND learners (and their families) who aspire to study at Bridge Training.
• Collate essential information about young people with SEND to ensure that support packages are appropriate and relevant once enrolled.
• Contribute to Bridge’s EHCP processes including annual reviews and legal consults.
• Develop and maintain excellent working partnerships with local schools and agencies.

You will also champion Bridge’s ethos of holistic, person-centred learning, and work with learners who may have complex personal barriers, including social, emotional, and mental health (SEMH) needs.

We’re Looking for Someone Who:

• Is passionate about transforming lives through education
• Has experience working with young people with SEND, and their families
• Can empathise with young people who may be facing challenges with engagement and how to provide support for them to overcome those.
• Competent understanding of SEND Code of Practice and processes such as annual reviews and legal consults.
• Is able to manage their own time and priorities effectively

At Bridge Training, you won’t just be taking on a job—you’ll be becoming part of a team. You’ll have the opportunity to:

• Make a genuine difference in the lives of young people
• Work within a values-led organisation where students come first
• Shape inclusive practices and help others grow
• Join a collaborative and supportive team that believes in change
