Job summary

We are very excited to offer an opportunity for a Band 5 or 6 Mental Health Nurse development post within the Stroud Assertive Outreach Team. The post is full time and flexible working can be discussed. You will be based at Weavers Croft in Stroud. The post would mainly involve working in and around Stroud and Cirencester with surrounding area's.

The Stroud and Cirencester Assertive Outreach Team is a friendly, welcoming and dynamic team providing secondary mental health care services to adults of working age, their families and significant others, to provide the best possible outcomes for people experiencing severe enduring mental illness, with complex mental health needs including dual diagnosis. The aim of the team is to work in line with the principles of the Assertive Outreach Model and Social Inclusion. We strive to ensure that locality services represent excellent quality, (safe and a positive experience) and good value, (efficient and effective).

The team provides a planned needs led service 9am-5pm five days a week.

Although occasional weekends maybe required on a clinical need basis.

Main duties of the job

This is an exciting opportunity for a mental health nurse who has some experience of working with adults of Working Age and caring for individuals with Enduring Mental Health problems. The post will include care coordinating a designated caseload, working within the Assessment and Care Management Policy and associated countywide safety procedures, administering depots, monitoring medication, supporting the Clozapine clinic, undertaking duty and initial assessments, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.
