Job summary
This post is critical to the development and delivery of effective and high standard Continuing Healthcare provision within Bristol, North Somerset and South Gloucestershire. The post holder will hold a caseload of highly complex individuals with learning disabilities and/or autism requiringbespoke case-management, and will be responsible for clinical case management, assessments and reviews of people known to the Continuing Healthcare team.
Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing individuals on the caseload of the team, and will use clinical reasoning and criticalthinking to effectively meet the needs of CHC funded individuals. The post holder will develop knowledge in relation to continuing healthcare, underpinned by theory and clinical expertise, and will work in partnership with other disciplines and agencies, including GPs, social services, community nursing and therapists, care agencies, care homes and mental health services in order to manage the care of people with highly complex needs, with a particular focus on Learning Disabilities and Autism.
Main duties of the job
Provide a skilled and specialist comprehensive assessment of care needs for and to clients.
Act as patient advocate for complex and vulnerable client group, with a main focus on individuals with Learning Disabilities and Autism.
Monitor and review care for own case load.
Undertake to provide personalised care plans where appropriate with consultation from other professionals.
Ensure care is appropriately delivered.
Liaise with acute sector, community services, social services, care agencies, nursing homes and voluntary agencies as appropriate.
Work in partnership with patients and carers to enable them to make informed choices about care and treatment to promote their health and wellbeing.
To work in a collaborative manner with, Health & Social care colleagues in the co-ordination and best interest of the client.
Carry out Mental Capacity Assessments relating to care and accommodation for the caseload.
Carry out Best Interest Meetings when appropriate.
Present cases to complex case panel for funding approval to ensure that care is safe, sustainable and equitable.
Ensure that robust systems for communication within the team are followed.
Maintain clear and concise and up to date records of care, in line with approved standards.
To ensure contemporaneous and accurate record keeping is maintained.
