As a Cardiac Rehabilitation Specialist Nurse, you will carry out comprehensive nursing and holistic assessments for patient with Coronary Heart Disease. Your responsibilities will include:
- Interpreting complex clinical information to inform care decisions.
- Identifying early signs of deterioration to help prevent hospital admissions.
- Assessing social circumstances and providing tailored health education.

Main duties of the job
We are seeking a highly motivated, enthusiastic nurse with proven specialist knowledge and experience to join our growing team!
This is an rewarding opportunity to make a meaningful impact on the clinical outcomes and quality of life for individuals recovering from Coronary Artery Disease (CAD). The roles involves acting as clinical expert, providing guidance and support to both primary and secondary care team, while working collaboratively with other community healthcare professionals to ensure seamless patient-centred care.
We're looking for someone with a strong background and in-depth knowledge of Coronary Artery Disease (CAD), who can apply their expertise to all aspects coronary heart disease management, including clinical assessment, service development and audit. Playing a key role in sharping and enhancing the quality of care we provide.
