Community Palliative Care Nurse Specialist, Band 6 (24 hours a week)

We are seeking a motivated and compassionate Community Specialist Palliative Care Nurse to join our dedicated specialist palliative care service in Gloucestershire. This is an exciting opportunity for an experienced nurse who is passionate about providing high-quality, person-centred centred care within a variety of community settings.

As a Community Specialist Palliative Care Nurse, you will play a key role in supporting patients with complex symptom management needs, enabling people to achieve their preferred place of care and supporting families through difficult and often life-changing circumstances.

Additional working pattern details: 9am - 5pm over 3 weekdays

Main duties of the job

You will work collaboratively with GPs, district nurses, hospital teams, hospices, care homes and wider health and social care partners to provide specialist advice, assessment and support for patients, and those important to them, who have complex symptoms and needs.

Key responsibilities include:

- Assessing and managing patients with complex palliative and end of life care needs.

- Providing specialist symptom management advice and support.

- Developing personalised care plans in partnership with patients and families.

- Supporting advance care planning and decision-making conversations.

- Offering education and support to healthcare professionals across the system.

- Contributing to multidisciplinary team meetings and care coordination.

Supporting the delivery of high-quality, equitable specialist palliative care across Gloucestershire.
