Job summary

The position is being offered on a permanent basis at 0.6 WTE, which equates to 22.5 hours per week.

We are seeking a committed and enthusiastic NMC-registered Band 5 nurse with community experience. The applicant will need to demonstrate excellent clinical and communication skills and demonstrate the ability to manage a patient caseload.

The community setting is an exciting, varied and complex environment in which to be a nurse, providing the fulfilling experience of caring for patients in their own homes whilst taking responsibility for the planning and implementation of holistic, patient-centred care.

This position is with the Severn Health Team which is based in Stonehouse. We are a family-friendly organisation. We currently work on a rolling rota for weekends, usually 1 in 4, supporting a good work/life balance.

Main duties of the job

Community nursing is a varied role that requiresautonomous working within a variety of settings including, but not limited to, patients' homes and Care Homes. The main duties are:

  • To Practice as a member of the community nursing team, working within the Integrated Community Teams (ICTs).
  • To liaise with GPs, other ICTs and statutory and voluntary agencies.
  • To provide advice, information and education to patients and carers with the aim of enabling patients to enjoy maximum possible independence, health and well-being.
  • To creatively manage the holistic and clinical needs of patients with acute, sub-acute and long-term conditions.
  • To enable patients living long-term physical health challenges to live well within their communities, with a focus on not only their physical health needs but also their wider holistic needs, e.g. mental health, learning disabilities, psychological needs, social needs and spiritual needs.
  • To excel in this role, you will require excellent communication skills, the courage to make clinical decisions and a willingness to develop your potential. You will require current NMC registration as a Registered Nurse (adult, physical health).
