The South Gloucestershire Recovery Outreach Service & Engagement (ROSE) Team is seeking an experienced and motivated Band 6 Specialist ROSE Practitioner - Care Coordinator to join a dynamic, psychologically informed service dedicated to supporting adults with complex mental health needs.
As part of the South Gloucestershire Recovery Team, you will work with individuals who are hard to engage, have high hospital admission rates, and experience severe and enduring mental illness. With a capped caseload of 15, you will have the opportunity to deliver flexible, creative, and person-centred interventions that improve service users' quality of life.
You will join a friendly, experienced, and supportive team committed to recovery-focused, holistic care and led by a Senior Practitioner.
Main duties of the job
Deliver assertive outreach and enhanced engagement interventions to individuals with complex mental health needs.
Undertake comprehensive mental health assessments using tools such as the Cluster Allocation Support Tool (CAST), Recovery Star, and KGV.
Act as a care Keyworker for complex cases and deliver defined therapeutic interventions, including:
CBT, DBT-informed work, and other psychological therapies.
Psychosocial and motivational strategies.
Medication management and physical health support.
Interventions under the Mental Health Act.
Build and maintain effective therapeutic relationships, promoting recovery and social inclusion.
Work collaboratively with families, carers, and multidisciplinary partners, including primary care, inpatient teams, and voluntary services.
Support the development of Personal well being plans, crisis and contingency plans with service users and stakeholders.
Lead, supervise, and mentor other staff and students in the team, contributing to service development and best practice.
Maintain accurate, timely electronic health records and ensure performance and safeguarding standards are met.
