We are recruiting to a part-time School and Community Health Facilitator role in Cirencester to work 18 hours per week over 3 days. You will work alongside the School Nursing team to

  • deliver the National Child Measurement Programme and Vision Screening
  • give advice and support to parents on a 121 and group basis who are concerned about their child's weight, sleep patterns, anxiety and other health issues
  • complete delegated packages of care

Main duties of the job

The successful candidate will work autonomously or as part of a team to provide screening programmes to children at primary schools in the Gloucester city locality to identify and address health needs in the local population. You will facilitate parent groups providing advice and support for health issues they are concerned about in their children. This may also include one to one work with families following a package of care.

Key responsibilities include

  • organising and delivering National Child Measurement Programme and vision screening
  • facilitating healthy lifestyles and parenting hubs including preparing resources and completing health records
  • make every contact count by promoting healthy lifestyles at each contact
  • deliver health promotion and education sessions for parents at school events
  • keep accurate and contemporaneous child health records

You will report to a school nurse in the Gloucester team and at times report directly to the team leader.
