Job summary

This role is offered on a fixed term/secondment basis for 12 months. The hours are part-time, 15 hours per week.

The Macmillan Next Steps Cancer Rehab Team are looking for an experienced and motivated Band 4 who is looking for a new challenge working within a well established multidisciplinary team, to holistically support and empower people affected by cancer to take an active role in their health and wellbeing. The post holder will support people affected by cancer through both 1:1 sessions (which may be face to face or virtual) and co-facilitating a wide range of health and wellbeing events and programmes throughout Gloucestershire. They will need to demonstrate the ability to facilitate group workshops plus listen, empathise and empower people affected by cancer to gain the knowledge, skills and confidence to recover after cancer and self-manage.

Main duties of the job

The postholder will undertake simple patient triage and holistic assessments, face to face or by telephone and assist in patient care and support activities in collaboration with the Next Steps Team.

They will holistically assess and contribute to self-management care plans and action plans, proactively involving and informing patients and utilising the weekly MDT.

The postholder will use coaching and patient centred care to support ways to maximise self-management strategies with patients. Assist in raising awareness of self-management, health and well-being, and how it can be best implemented in a 1:1 or group setting.
