Band 6 Mental Health Practitioner - Swindon
Swindon
Job summary
Are you a Mental Health Nurse or Social Worker, looking for an opportunity to work within community mental health services?
We have an opportunity for both experienced Band 5 practitioners, or newly qualified Band 5 practitioners with minimum of 6 months experience in post qualifying post (successful 6 months ASYE or preceptorship sign off), to support their transition to Band 6 role.
This post is open to newly qualified social workers. In line with trust policy, all newly qualified social workers will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSWs are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be agreed to extend this for a further 2 years.
On successful completion of the ASYE, for which you will be given full support of an experienced Social Worker as well as Divisional Social Work Lead, and peer support from other ASYE candidates, you will then be eligible to progress to a Band 6 position.
Main duties of the job
We are a multidisciplinary team consisting of a talented pool of both registered and non-registered mental health professionals.
We have moved away from the use of care programme approach (CPA) and now work with service users and carers under the "your team, your conversation, you plan approach", which is a strength based, intervention lead model of care, that supports the individual to address their own identified needs.
Applicants will need to demonstrate knowledge in the areas of the delivery of mental health interventions in the community, with particular reference to the needs of Older People, together with an understanding of both the PWP and Care Management processes.
You will be required to provide key worker interventions for an allocated caseload of service users with substantial and complex mental health needs. This will include key tasks of undertaking high quality assessments, developing care plans, delivering and reviewing therapeutic interventions. You will act as a supervisor and role model to others.
Applicants will be expected to demonstrate a high level of decision making skills and to work independently.
Applicants will also need to be proficient with the use of IT systems for both data and documentation purposes.
