Accountable to: Operations Manager

About the Role

We are seeking a reliable and well-organised Care Co-ordinator to join our administrative team within primary care. This is a predominantly administrative role, providing essential support to clinical and non-clinical staff to ensure the smooth, efficient running of practice systems and processes.

The post holder will focus on managing clinical administration, supporting patient recall and follow-up processes, maintaining accurate patient records, and assisting with the coordination of care through effective administrative support. While the role contributes to patient care, it does so through robust administration rather than clinical decision-making or hands-on care.

Key Responsibilities

  • Provide administrative coordination and navigation support across primary care and PCN services, working closely with clinicians and PCN colleagues
  • Support clinical workflows by filing results, managing clinical correspondence, allocating tasks, and maintaining accurate records using SystmOne
  • Work closely with Social Prescribing Link Workers, Health and Wellbeing Coaches and other PCN colleagues to support the coordination and administration of patient care
  • Accurately code, summarise, scan, and index patient records in line with practice protocols
  • Manage patient recalls, routine monitoring, and follow-up actions (e.g. blood tests, long-term condition reviews, NHS Health Checks, QOF activity)
  • Contact patients as required for administrative follow-up, appointments, and reminders
  • Support patients by managing queries, making and coordinating appointments, and ensuring patients receive appropriate written or verbal information to support their care.
  • Assist with managing incoming and outgoing clinical and administrative correspondence
  • Support engagement with hard-to-reach patients through structured administrative processes
  • Assist with the promotion and administration of practice initiatives, including NHS App support and health campaigns
  • Provide administrative cover for colleagues during periods of annual leave
  • Support ordering, stock control, and general office administration across sites
  • Maintain a tidy, organised, and confidential working environment

Training and Development

  • Full training, clinical supervision, and leadership support provided
  • Regular one-to-one meetings and annual appraisals
  • The post holder will be enrolled in, undertaking, or supported to complete appropriate Care Co-ordinator training in line with the Personalised Care Institute Workforce Development Framework

About You

You will be an organised and detail-focused individual with strong administrative and IT skills, comfortable working in a fast-paced healthcare environment. You will be confident managing multiple tasks, following procedures, and handling sensitive information appropriately.

Essential requirements include:

  • Good general education including GCSE Maths and English (or equivalent)
  • Experience working with the public and undertaking administrative duties
  • Strong IT skills and confidence using email and office systems
  • Excellent communication, organisational, and time-management skills
  • Ability to handle confidential information appropriately
  • Flexibility to meet service needs
  • A clear DBS check and right to work in the UK

Desirable experience and qualifications include:

  • Experience working in primary care or a healthcare setting
  • Experience using SystmOne
  • AMSPAR, NVQ Level 2 in Health & Social Care, or relevant IT qualification
  • Experience in care coordination or personalised care planning
