Technology & Digital Inclusion Social Prescriber Link Worker

GRCC (Gloucestershire Rural Community Council) working in partnership with
North Cotswold Primary Care network

Are you passionate about using technology to improve people’s lives?

We are looking for a Technology & Digital Inclusion Social Prescriber Link Worker to join
GRCC’s award-winning DAISI Project team, working alongside GP practices across the
North Cotswolds to support patients who face barriers linked to isolation, long-term
conditions, disability, mental health, or digital exclusion. You will work to help them
access practical human and technology-enabled solutions that improve their lives.

You will:

• work directly with patients referred by GP practices and partners;
• co-produce personalised support plans using digital assistive technology;
• help people to build confidence with devices, apps, and online services;
• connect patients to healthcare, community, and voluntary support;
• champion inclusive, accessible technology that works for everyone.

The successful candidate will be:

• good with people;
• patient and able to listen;
• confident using everyday technology, with the ability to explain things simply;
• passionate about reducing health inequalities and digital exclusion;
• have an interest in working across the healthcare, community, and voluntary sectors.
