GRCC (Gloucestershire Rural Community Council) working in partnership with
North Cotswold Primary Care network
Are you passionate about using technology to improve people’s lives?
We are looking for a Technology & Digital Inclusion Social Prescriber Link Worker to join
GRCC’s award-winning DAISI Project team, working alongside GP practices across the
North Cotswolds to support patients who face barriers linked to isolation, long-term
conditions, disability, mental health, or digital exclusion. You will work to help them
access practical human and technology-enabled solutions that improve their lives.
You will:
- work directly with patients referred by GP practices and partners;
- co-produce personalised support plans using digital assistive technology;
- help people to build confidence with devices, apps, and online services;
- connect patients to healthcare, community, and voluntary support;
- champion inclusive, accessible technology that works for everyone.
The successful candidate will be:
- good with people;
- patient and able to listen;
- confident using everyday technology, with the ability to explain things simply;
- passionate about reducing health inequalities and digital exclusion;
- have an interest in working across the healthcare, community, and voluntary sectors.
