Domiciliary Care Coordinator

Job Purpose
The Care Coordinator is responsible for organising, monitoring, and supervising the delivery of high-quality care services to clients in their own homes. This includes planning staff rotas, supervision of care staff, and ensuring services are delivered in compliance with regulatory standards and organisational policies. This is not an exhaustive list of duties.
Key Responsibilities
Scheduling & Rota Management
-Develop and manage effective weekly rotas, ensuring continuity of care.
-Match appropriate care workers to Service Users based on individual requirements and compatibility.
-Handle last-minute changes due to staff absences, emergencies, or changes in Service User conditions.
-Ensure travel time and mileage is kept to a minimum
-Issue rotas on a fortnightly basis
Staff Support & Supervision
- Provide ongoing supervision, support, and mentoring to care staff.
-Conduct regular office supervision with care staff and participate in team meetings.
- Participate in the induction, and training of care workers.
Compliance & Quality Assurance
- Ensure all care services meet or exceed CQC standards and internal quality policies.
- Maintain accurate records of care delivery, staff performance, incidents, and concerns.
- Report safeguarding concerns and support with internal investigations and audits.
Client and Family Liaison
- Act as a key contact for clients and families, addressing queries, feedback, and complaints in a timely and professional manner.
- Promote open communication and transparency with all stakeholders.
Team Collaboration
- Work closely with the Head Care Coordinator, Registered Manager and other coordinators to ensure smooth operations.
- Attend team meetings and contribute to service improvement initiatives.
- Participate in the out-of-hours on-call rota.
Working Conditions
- Office-based with occasional community visits
- Participation in a shared on-call rota (including evenings and weekends)
- Fast-paced environment with responsibility for reactive scheduling and service delivery
Salary: £29,000
Hours Per Week: 40
