Job summary
The Advanced Practitioner (AP) provides high quality, individualised clinical management for patients using advanced clinical assessment and diagnostic reasoning skills to determine and initiate changes to treatment as necessary. The post holder will be a clinical expert in the field and will fulfil the 4 domains of Advanced Practice:
- Clinical Practice Leadership and Management Facilitation of Learning and Education Research and Development
Advanced practice is delivered by experienced, registered health and care professionals. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a master's level award that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence. Advanced practice embodies the ability to manage clinical care in partnership with individuals, families and carers. It includes the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes. This definition therefore requires that health and care professionals working at the level of advanced practice will exercise autonomy and decision making in a context of complexity, uncertainty and varying levels of risk, holding accountability for decisions made.

Main duties of the job
- Act as a clinical expert in a specific area of direct contact care, able to make high level skills decisions, informed by advanced diagnostic, assessment and treatment skills. To be responsible and accountable for own decisions, actions and omissions at this level of practice
- Critically reflects on their own practice and acknowledges limitations in knowledge, referring to senior clinicians when scope of practice is reached
- To appropriately assess, examine, investigate, diagnose and treat patients resulting in the safe management of patients
- Able to use decision making skills to inform clinical reasoning approaches to manage differentiated and undifferentiated presentations and complex situations to make evidence-based judgements
- To assess the effectiveness of interventions and advise ongoing management or refer on to appropriate clinical specialists if necessary
- Develop scope of advanced clinical skills and practice to meet changing needs of service
- Prescribe, supply and /or administer medication within appropriate legal frameworks and Trust guidance.
- Facilitate 'expert' inter-professional care for the patient pathway and their dependants by direction, supervision and example
