Care Coordinator - Part Time


An exciting opportunity has arisen to join our friendly, dedicated multi-disciplinary navigation team at Rosebank Health as a Care Coordinator. We currently serve a population of over 39,500 patients in four locations; Rosebank Surgery, & Bartongate Surgery based in the city centre of Gloucester, Kingsway Health Centre and Severnvale Surgery at Quedgeley.

The Job:

Care Coordinators review patients’ needs and help them access the services and support they require to understand and manage their own health and wellbeing.

To Join the Navigation team you need to be:

The role of a Care Coordinator is not a clinical role but involves a lot of patient contact and you will need good interpersonal & communication skills, patience and diplomacy.

We have a part time vacancy available for 32 hours per week, fixed hours over 4 days Monday – Friday between the hours of 8.00am & 6.30pm. The role will predominantly be based at Rosebank surgery however the successful applicant will need to be prepared to work at all four surgery sites.  Rate of pay dependent upon experience.

You will need to follow the surgery’s systems, policies and procedures carefully and ensure that you maintain patient confidentiality at all times.

You will be trained in Care Navigation and there will be opportunities for development and progression within the surgery.

About You:

You will need to be able to work and think clearly under pressure with competing distractions. You need to be committed to enhancing the experience of our patients & be skilled at patient navigation & problem solving.

You will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. You will have good written and verbal communication skills and strong organisational and time management skills. You will have a flexible attitude, keen to work and learn as part of a team and committed to providing our patients with high quality support.

The role of a Care Coordinator will require the post holder to provide an enhanced patient navigation service. You must be comfortable with computers. Previous experience working in a GP practice is preferred but not required. Full training will be provided.

Experience in customer service fields and second languages may be helpful in your application.
