Patient Care Coordinator Supervisor
Gloucester
An exciting opportunity has arisen to work as part of our hub navigation team at Rosebank Health as a Patient Care Coordinator Supervisor.
Care Coordinators review patients’ needs and help them access the services and support they require to understand and manage their own health and wellbeing
The role of a Care Coordinator Supervisor involves a lot of patient contact and you will need good interpersonal & communication skills, patience and diplomacy. You will need to be able to work and think clearly under pressure with competing distractions. You need to be committed to enhancing the experience of our patients & be skilled at patient navigation & problem solving. You will be required to support a team of Care Coordinators alongside the Senior Care Coordinator and provide assistance to our clinical team.
About you
You will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. You will have good written and verbal communication skills and strong organisational and time management skills. You will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.
The role of a Care Coordinator Supervisor is not a clinical role and will require the post holder to provide an enhanced patient navigation service, above and beyond the patient navigation currently carried out by our reception team. Full training will be provided.
There will be opportunities for development and progression within the surgery.
You will need to follow the surgery’s systems, policies, and procedures carefully and ensure that you always maintain patient confidentiality.
The role will predominantly be based at Rosebank surgery however the successful applicant will need to be prepared to work at all four surgery sites.
The Care Coordinator Supervisor role is 30-37.5 hours per week on a flexible working basis.

Salary: Starting rate of pay : £11.75 per hour
