Job summary

This role is offered on a permanent basis. The hours are part-time, 30 hours per week.

As part of community mental health transformation we are seeking to develop the community rehabilitation pathway across Gloucestershire Health and Care NHS Foundation Trust with a particular focus on individuals with treatment resistant psychosis and frequent admissions or long stays in inpatient units locally, and out of county, as well as individuals with 24 hour support whose placements are at risk of breaking down.

This newly established team will seek to build links between internal services, VCSE (voluntary, community or social enterprise) organisations and existing/new care providers to ensure a multi-agency, collaborative and recovery-oriented approach to complex rehabilitation. This will include creating a shared ethos and goals and a sense of hope and optimism to deliver individualised and person-centred care to promote independence and integration within the local community.

Main duties of the job

As Mental Health Rehabilitation Co-ordinator the main duties will be:

  • To support the multi-disciplinary team by determining recovery orientated goal plans including identifying recovery focussed activities and fostering a culture of hope
  • Develop, implement and evaluate care plans in conjunction with service users, carers and other professionals which reflect individual needs
  • Provide assessment and implementation of evidence-based interventions for service users who are experiencing common, acute mental health and longer-term recovery needs
  • To work alongside a number of service users on a one to one or group basis, potentially within the individuals own home, or support junior staff, providers and VCSE organisations to carry out this work effectively
